In order to participate in extra-curricular athletics in Cobb County, ALL athletes must have a physical on file, dated April 1st or after for the next season.

Please be sure to visit the CCSD Athletics Website where you can download the:

- Physical Exam Form that you must print and have filled out by an approved professional. You DO NOT need to have this filled out by your Primary Care Pysician. Many walk-in clinics offer low cost physical exams, just use the County Form.

You must also complete your athletes Athletic Registration in ParentVue under 'Athletic Registration'. In this portal, you will upload pictures of each page of the CCSD sports physical, answer other health questions and check off on some additional paperwork.
